Samaritan Hospice provides specialized medical care and emotional support for people facing serious and progressive illness in the comfort of their homes or other residences. Samaritan offers an interdisciplinary team of nurses, social workers, home health aides, spiritual support coordinators, bereavement counselors and volunteers to help patients and their families live as fully and comfortably as possible through individualized plans of symptom management and emotional support. Services are provided regardless of financial circumstances. Samaritan has been an innovative provider of hospice care since its inception in 1980. It is now one of the 15 largest providers of hospice care in the country. It has cared for more patients than any other hospice in the Middle Atlantic States and remains on the cutting edge of hospice care.

Mission: Meals On Wheels of San Francisco and its volunteers provide seniors what they need to live independent and dignified lives — nutritious meals, professional social work and a friendly visitor. Our network of drivers, social workers, nutritionists, community partners, volunteers, donors and advocates are a formidable force against senior neglect and malnutrition. Citymeals on Wheels mission is to provide a continuous lifeline of nutritious food and human contact to homebound elderly New Yorkers in need, thereby helping them live with dignity in their own familiar homes and communities.

India Home Inc. is a not-for-profit organization in New York started in 2007 by healthcare providers who personally and professionally witnessed the social, psychological and medical problems of seniors. India Home is committed to providing services for seniors in culturally sensitive environment by collaborating with other not for profit organizations, individuals, health care professionals, and government organizations. Our goal is to provide quality care by adding life to the years in a constantly collaboratively environment. After seven years of providing senior center services in a culturally and linguistically appropriate environment once a week in different parts of Queens through collaborations, we opened the Desi Senior Center one year ago in Jamaica providing three days a week senior center services. As a result of this expansion, we currently serve over 150 individual seniors every week and provided over 20,000 congregate meals to South Asian seniors in just the past one year!

Homesteads for Hope is a unique Community Farm experience for transitioning young adults with and without disabilities providing opportunities to learn, work, live and grow in the Greater Rochester Region. Our membership, programs, services and inclusive events are hosted on a 55 acre farm located on the banks of the historic Erie Canal creating a venue of dignity, purpose and sustainable agri-tourism.
