The cornerstone of hospice and palliative care is the belief that individuals and families coping with incurable and progressive illness should be cared for compassionately. Hospice by the Bay's mission is to alleviate pain; to prevent and treat conditions that might otherwise threaten the quality of life; and to provide support to the patient's family and loved ones, including grief counseling for adults, teens and children.

Cerenity Senior Care is a faith‑based senior care organization that provides a continuum of services (assisted living, memory care, skilled nursing, rehabilitation and related supports) intended to preserve residents’ dignity, independence and well‑being. Their work emphasizes compassionate, values‑driven care rooted in the Benedictine mission of hospitality, stewardship, respect and service to vulnerable older adults.

THE FOUNDATION OPERATES EXCLUSIVELY TO PROVIDE SUPPORT FOR THE PROVISION OF NURSING SERVICES AND HOSPICE CARE SERVICES BY VISITING NURSE SERVICE AND HOSPICE OF SUFFOLK, INC. (HEREINAFTER,"VNSHS"), A NOT-FOR-PROFIT ORGANIZATION THAT IS A CERTIFIED HOME HEALTH CARE AND HOSPICE AGENCY, DEDICATED TO PROVIDING INDIVIDUALS WITH HIGH QUALITY NURSING, THERAPY, AND HOSPICE SERVICES IN THEIR PLACES OF RESIDENCE AND WITHIN AN INPATIENT HOSPICE FACILITY.

Friendship Home provides overnight respite care, social-recreation programs and vocational/individual support services for people with developmental disabilities and their families on the South Shore of Massachusetts. Its programs (including an overnight “Home Away From Home,” the Bridges to Work vocational program, and Friendship Club social activities) aim to build life skills, community inclusion, independence and meaningful social connections.

Meals on Wheels Durham delivers daily nutritious meals plus friendly wellness checks to homebound older adults and other community members in Durham County so they can remain healthy, independent, and connected. The organization also provides related supports (holiday gifts, pet food, discharge meal programs, etc.) to improve clients’ overall wellbeing.

Open Arms prepares and delivers medically tailored, made‑from‑scratch meals and provides nutrition support to people living with serious illnesses and their families, at no cost to clients. They combine dietitian‑guided menus, home delivery, and volunteer support to help clients focus on healing.

JCHE provides safe and affordable independent housing where older adults of all backgrounds can age in community. Our programs and services enable residents to live a full life of connection and purpose in a dynamic and supportive environment.
