Aging Services for Communities is a Minnesota non‑profit that provides in‑home supports (transportation, homemaking, and companion services) so older adults, people with disabilities, and families can remain independent in their own homes. The agency focuses on practical, community-based help (rides to medical appointments, homemaker assistance, socialization and companion care) across several southern Minnesota counties.

CHENEY GOLDEN AGE HOME, INC. IS DEDICATED TO THE CONCERNS OF OUR RESIDENTS. OUR PRIMARY CONCERNS ARE THE NEEDS , INTERESTS , AND ABILITIES OF EACH INDIVIDUAL RESIDENT. THIS INCLUDES PROVIDING QUALITY CARE IN A CARING ENVIRONMENT AND MAINTAINING RESIDENTS AT THEIR HIGHEST PRACTICABLE LEVEL OF FUNCTIONING.

Founded in 1966, the mission of Cooperative Metropolitan Ministries, Inc. is to mobilize congregations and communities across economic, religious, racial, and ethnic boundaries so that, in partnership, we can work more effectively for a just and peaceful society and for spiritual growth and interfaith understanding.

Founded in 1974 on the belief that no one should be hungry or alone in a world of caring people, Caring for Friends is a volunteer organization and independent food bank serving greater Philadelphia. CFF provides food and friendship to homebound seniors, veterans, and those with disabilities; delivers meals to people facing homelessness; and donates food to more than 250 community partners in unserved and underserved neighborhoods. In 2021, CFF rescued, sourced and distributed more than 12 million pounds of food. Volunteer to cook meals, pack snacks, make deliveries or visit with clients at caringforfriends.org.

Realizing that people can be impoverished physically, mentally, emotionally, spiritually, socially and financially, Covenant Soup Kitchen provides food and access to basic services in an environment of care, love, support and safety to those individuals and families from the greater Windham community who come through our doors. We rely on the generosity of the community and its faith to help us carry out this mission.

The mission of Mystic Valley Elder Services is to support the rights of older adults and adults living with disabilities to live independently with dignity in a setting of their choice by providing them and caregivers with information, advice, and access to quality services and resources.

BAYADA Home Health Care has a special purpose — to help people have a safe home life with comfort, independence, and dignity. BAYADA Home Health Care provides nursing, rehabilitative, therapeutic, hospice, and assistive care services to children, adults, and seniors worldwide. We care for our clients 24 hours a day, 7 days a week.Families coping with significant illness or disability need help and support while caring for a family member. Our goal at BAYADA is to provide the highest quality home health care services available. We believe our clients and their families deserve home health care delivered with compassion, excellence, and reliability, our BAYADA core values.

ROSARY CARE CENTER OPERATES A LICENSED NURSING FACILITY AND A LICENSED RESIDENTIAL CARE FACILITY. THE NURSING FACILITY IS OPEN TO BOTH RETIRED MEMBERS OF RELIGIOUS ORDERS AND RETIRED LAY PERSONS. THE RESIDENTIAL CARE FACILITY IS OCCUPIED BY RETIRED MEMBERS OF THE SISTERS OF ST. FRANCIS. ROSARY CARE CENTER PROVIDES SKILLED NURSING CARE AND PERSONAL CARE SERVICES IN BOTH FACILITIES TO SICK, INFIRM, CONVALESCENT AND AGED PATIENTS UNDER THE DIRECTION OF PHYSICIANS AND OTHER HEALTH CARE PROFESSIONALS. THE SISTERS OF ST. FRANCIS CONTROL ROSARY CARE CENTER THROUGH SYLVANIA FRANCISCAN MINISTRIES, AN OHIO NONPROFIT CORPORATION WHICH ACTS AS THE PARENT OF ROSARY CARE CENTER. THE CONGREGATIONAL MINISTER AND MEMBERS OF THE GENERAL COUNCIL ARE THE MEMBERS OF SYLVANIA FRANCISCAN MINISTRIES AND APPOINT SYLVANIA FRANCISCAN MINISTRIES BOARD OF TRUSTEES.

We see a ministry where the lives of the homeless and those in need are constantly being transformed by the truth of the Gospel with the collaboration of our community.
