REACH is a community health centre that has been located on the east side of Vancouver since 1969. We became incorporated as a non-profit society in 1970 with a Board of Directors comprised primarily of community members and patients of the clinic. Since its inception, REACH has endeavored to meet the medical, dental, and cultural needs of the community with an emphasis on prevention and the development of programs which reflect the ongoing needs of the ever changing composition of the community. We are committed to reducing health inequities. The community of which REACH is a part is a richly diversified ethnic setting in the area known as Grandview Woodlands. REACH has a strong commitment to community development and works to build a collaborative relationship with users of the clinic. Recovery Acres is a 34 bed treatment facility in Edmonton, Alberta helping adult men recovery from alcohol and drug addiction. We provide a safe, secure and supportive environment for those who wish to make the commitment and put in the work necessary to change their lives. Through life skills and addiction awareness modules, individual counselling and 12-step work, Recovery Acres helps men through the early stages of addiction recovery, building relapse prevention skills and the community supports necessary to eventually transition back into the community.
Ontario March of Dimes is one of the largest charitable rehabilitation organizations in Ontario, providing a wide range of services across the province to enhance the independence and community participation of people with physical disabilities. Our consumers include those who have cerebral palsy, spina bifida, muscular dystrophy, multiple sclerosis, Parkinson’s disease, post-polio syndrome, and people who have acquired their disability through injury, or have experienced a stroke. We help people find jobs, acquire wheelchairs and other assistive devices, provide recreation programs to those who are socially isolated, and so much more.

Rosalie Hall has been serving Toronto communities since 1914. Over 2500 young mothers and children are served each year. In Canada, approximatley 33,000 adolescents become preganant each year. In Toronto alone, reported rates of childhood abuse in pregnant adolescents are as high as 52% for sexual abuse, 74% for physical abuse and 78% for emotional abuse.(Study by Uiversity of Toronto & The Hospital for Sick Kids in collabration with Rosalie Hall) Rosalie Hall, together with our supporters aim to break intergenerational cycles of abuse and poverty, that often lead to a young mother's present situation. Rosalie Hall provides its services with compassion and non-judgment. We hope to educate others on the great needs of these vulnerable young families. We recognize the harsh realities that instability, abuse, violence, and poverty have on fragile lives and provide our programs and services to address the complex lives of many of the young parents served.
In 1976, Metro Toronto Legion Village welcomed its first 14 residents. It was renamed Tony Stacey Centre for Veterans Care in 2003 in honour of one of its founders. Over 600 residents have called it home. The centre is built on two floors, in the shape of a cross. It has 50 rooms, 100 beds, and provides long-term care for married couples or single residents. We also welcome Silver Cross Mothers, mothers of veterans who have died in combat. The average age of our residents is 82. The centre is a complete community. It has lounges, reading areas, arts and crafts rooms, a therapeutic swimming pool, a bar and a gift shop. Residents can get their hair done in the beauty salon, attend meals in the dining room, and find solace in the chapel. In addition to management staff, the Centre’s team of professionals includes Registered Nurses, a Medical Director, a Dietitian, a Physiotherapist, a Social Worker, a Massage Therapist and many more.

Nestled in a peaceful setting near Riley Park in northwest Calgary is The Salvation Army Agapè Hospice. It is a home away from home for the terminally ill, offering people a place where they can die with dignity. It is also a place that echoes with the hope of life. Compassionate and professional care is the backbone of the hospice. The philosophy: caregiving is most effective when residents, health professionals, volunteers, and family work as a team. Teamwork is what makes Agapé special. Since The Salvation Army opened the hospice in 1992, thousands of individuals, including families and friends, have benefited from the nursing and spiritual care, family support, pain management, and bereavement services that Agapé Hospice offers.
